POSITION TITLE: Hospice Care Consultant

REPORTS TO: Access Patient Care Manager

FLSA: Exempt

Position Scope:

The Hospice Care Consultant, under the direction of the Access Patient Care Manger, sells hospice services in a defined market or territory. Sales activities include account identification and development, servicing and growing facility, physician and in some instances home health agency relationships within a defined territory. All sales activities are in accordance with the business plan strategy for the market.

I. Essential Functions Statement(s)

  • Identifies, profiles and calls on key physicians, skilled nursing facilities, rehabilitation facilities, assisted living facilities, home health agencies, and other senior living settings within the assigned territory to open new accounts and promote hospice referrals. o Track and trend referral patterns. o Maintains detailed territory list, prioritizing accounts in accordance with the market/territory sales plan. Maintains detailed territory management systems records as required. o Work with all teams to ensure high level of customer satisfaction. o Handle objections and concerns from referral sources promptly and professionally. o Communicate with physicians, skilled nursing facilities, rehabilitation facilities, assisted living facilities, home health agencies, and other senior living settings in a professional manner. o Utilize creative ideas and strategies to enhance utilization Valley Hospice and Caring

Connections services. o Participates in agency sponsored events as indicated, e.g. volunteer recognition, bereavement activities, fundraising events, community events, e.g. Relay for Life,Alzheimer’s memory walk or other events as requested.

  • Assists Marketing Director and Access Patient Care Manager in developing and executing plans to increase utilization of Valley Hospice and Caring Connections services and generate referrals from physician offices and the community. o Utilize a scheduled call rotation to actively engage referral sources, assisting them in identifying patients eligible for hospice or palliative care services, resulting in an expected rate of Valley Hospice and Caring Connections referrals. o Maintain accurate records of prospective and active accounts. o Submit itineraries and weekly call reports in a timely manner. o Meet or exceed monthly goals for referrals.

o Meet minimum call standards through contacts, group in-services and pre-determined community events.

  • Collaborates with Marketing Director and Access Patient Care Manager on development and implementation of presentations, handouts and strategies for growth. o Collaborate with Marketing Director and Access Patient Care Manager on content and collateral used in presentations and handouts. o Collaborate with PCM’s to communicate patient issues and challenges experienced with targeted physicians, skilled nursing facilities, rehabilitation facilities, assisted living facilities, home health agencies, and other senior living settings. o Collaborate with Access Patient Care Manager and Caring Connections APN to communicate patient issues and challenges experienced with targeted physicians, skilled nursing facilities, rehabilitation facilities, assisted living facilities, home health agencies, and other senior living settings .
  • Represents Valley Hospice at community events, to community and professional organizations. o Attend health fairs and community events as assigned, including some evenings and weekends. o Act as a role model and change agent for staff at Valley Hospice. o Uses agency resources wisely. o Participates in performance improvement activities.

Education and Work Experience:

Education: Bachelor's Degree (four year college or university), Degree in marketing, public relations or related field preferred required.

Experience: Two to four years related experience

WORK ENVIRONMENT
While performing the duties of this job, the employee frequently is exposed to outside weather conditions when traveling between sites. Patients’ home environments are variable with regard to heat, noise, air circulation and conditioning, space, cleanliness, accessibility and all other environmental factors. Inpatient and skilled nursing facility environments are standardized. Office environment in a controlled atmosphere building.

Job Type: Full-time

Pay: $58,000.00 - $68,000.00 per year
